BSA medical forms should be completed each year and the original kept by the family; two clear photocopies should be provided to the Troop. One copy will be used for summer camp, the other copy used by the Troop to comply with BSA requirements for certain activities (ie, Canoeing, Climbing, or strenuous) or when the unit will be more than 30 minutes from a road (ie, hiking, canoeing).

The Council also requires each person in camp to be checked in by the Camp Health Officer upon arrival. All Scouts and adults registered in camp MUST have a completed BSA medical and immunization form signed by their family doctor. Anyone without these records cannot remain at camp. Due to state law and BSA requirements, NO OTHER FORMS CAN BE ACCEPTED,.

Notes on the medical form:

  • Requires physical exam within 24 months (updated yearly);

  • tetanus shot within the past ten years.

  • Completed and signed by a doctor or licensed health-care practitioner.

  • Signed by a parent or guardian.

  • Adults over 40 require a yearly physical.
